Please obtain and bring the following with you to allow formulation of a personaliSed treatment plan for you. 

1. A current referral letter from your doctor and fax it to 03 9448 8012 before your appointment.

2. A list of your current medications (including dosages).

3. Any relevant blood test results (if available).

4. Any relevant reports and films of X-Rays, MRI, CT-Scan, Ultrasound and other imaging (if available).

Fees

• Our consultation fees are referenced to the Australian Medical Council List of Medical Services and Fees updated on 1st November 2015.

• To allow you to know the cost of your consultation before your appointment, our fee schedule is available on request. Fees chargeable will vary depending on the length and complexity.

• During your visit, Medicare claim will be processed on your behalf and the rebate refunded to the bank account linked to your Medicare card within 24 hours.
