Please obtain and bring the following with you to allow formulation of a personaliSed treatment plan for you. 

1. A current referral letter from your doctor and fax it to 03 9448 8012 before your appointment.

2. A list of your current medications (including dosages).

3. Any relevant blood test results (if available).

4. Any relevant reports and films of X-Rays, MRI, CT-Scan, Ultrasound and other imaging (if available).

Fees

• Our consultation fees are below the Australian Medical Council List of Medical Services and Fees updated on 1st November 2022.

• To allow you to know the cost of your consultation before your appointment, our fee schedule is available on request. Fees chargeable will vary depending on the length, complexity and location of the service.

• During your visit, Medicare claim will be processed on your behalf and the rebate refunded to the bank account linked to your Medicare card within 24 hours.
